HOW TO REGISTER WITH THE PRACTICE

You can register as a patient with the Practice either by completing a registration form which can be obtained from reception.  We would recommend that you come into register between 12:30 and 14:00 as this is usually the quietest period for the reception staff. Please try to attend between these times where possible. Reception staff may ask you to have a seat and wait if you attend during a busy period before they can dedicate the time needed to check your registration form.

It usually takes around 5 working days for your registration to be processed, to avoid any delays please ensure you complete the entire form to the best of your knowledge. Forms may be rejected if important information is missing.  

You will be asked to complete a questionnaire which will provide us with important information regarding your health.  Newly registered patients are also recommended to attend for a health check with the Health Care Assistant.  The health check is a method of introducing you to the Practice and our services.

If you are new to the country and Victoria Medical Centre will be your first GP surgery we also need to see and take a copy of a form of photo ID, e.g. a passport or ID card.
